How to use the PDF documents with Adobe Reader
Adobe Reader is a FREE application that allows you to open and fill the forms from your computer.
You must have the FREE application, Adobe Reader to view and use these forms.

To download the application, go to " www.adobe.com", and look for the icon "Adobe Reader.

| - Follow the instructions to download the application. |
| - Once the application has been installed on your computer, you will be able to fill the forms. |
| - Go back to the forms page and click the link for the form you want to use. The form should download and automatically open. |
| - Move your cursor (hand icon) over each line and if it changes to the flashing icon text "I" then that field is fillable. |
| - Type in your information. |

- Please note that certain fields will not be fillable as they may require special attention such as signatures.
|
| - Print out the form, and get the necessary signatures before submitting it. |
| Note: for security purposes this form cannot be saved. Please make a copy for your personal records. |
